This forms website has been compiled by request of the undergraduate academic advisement community, and its primary audience is the academic and student services support staff at UB. This effort has been a collaboration amongst various Undergraduate Education (VPUE) units, the Division of Student Affairs, and the Graduate School.
Ultimately, we hope to provide a site that will provide links to related policies and information, and will include the unit responsible for the form.
